Weather-Related Assistance Requests (ICS Form 213)
Posted November 2, 2026
During a declared weather emergency, community members can request assistance through Teesto Chapter. The chapter serves as your point of contact with the Navajo Nation Emergency Operations Center (EOC).
How to request assistance
- Contact the chapter by phone, email, or in person and describe your need. Include your name, phone number, and location.
- Chapter staff will complete the Assistance Request Intake Sheet with you and prepare an ICS Form 213 on your behalf.
- The Chapter Manager, as Local Incident Commander, approves the form and the chapter submits it to the NN EOC Planning Section.
- The EOC reviews the request and responds to the chapter.
- The chapter contacts you with the response or resources provided.
You do not need to complete any form in advance. The intake sheet is posted here so you can see what information we will ask for. You are welcome to print it and bring it in.
